What counties are the Wellmark Synergy Health Plans available in?

The Wellmark Synergy Health Plans are available for Iowa residents residing in Linn, Johnson, Scott, and Des Moines counties.

The Synergy Health network is made up of a number of qualify, local providers that partnered with the University of Iowa Health Alliance. Other counties may become available in the future, but the current plan availability is based on the location of providers and facilities.
