How do I cancel a Medicare Advantage Plan or Part D plan after a member’s death?

If you report death to Medicare/Social Security it will automatically term Part D or MAPD plan.

You still have to manually call and cancel Med Supplement plans.

To report the death of a person with Medicare:

Make sure you have the person’s Social Security Number.
Call Social Security at

1-800-772-1213 (TTY: 1-800-325-0778)
